What Paperwork Do I Need?
Application Checklist
|
Personal Data: Please provide Social Security Numbers
and phone numbers for all applicants.
|
|
Address History: Please provide information about
the most recent two years . If renting, provide landlord's name
and address. If you presently own or have owned a home, provide
lender's name, address and account numbers.
|
|
Assets: Please provide names, addresses, account
numbers, balances and copies of the most recent three months statements
from all accounts at banks, savings and loans, and credit unions.
Also provide information on stocks and bonds, automobiles and
real estate (description and value).

|
Debts: Please provide names, addresses, account
numbers, balances and amount of monthly payment on all auto loans,
notes to banks, finance companies, credit unions, retail stores,
etc.
|
|
Other Obligations: Child support/alimony - please
provide a copy of the divorce decree.
|
|
Employment History: Please provide the past two
years of employment history with complete names and addresses
of employers, and most recent paycheck stub and W-2's from past
two years. If self-employed, provide profit and loss statements
and a balance sheet for the current year prepared and signed by
an accountant. Alos, please provide the past two years signed,
complete federal income tax returns, including all schedules.

Other Income: Alimony/child support - please provide
a copy of court history or cancelled checks for past 12 months.
(You need not disclose income from these sources if you do
not want the income to be used to qualify for this loan)

Legal Documents: Bankruptcy - please provide a
copy of discharge and schedule of debts and assets.
|
|
Veterans Only: Certificate of Eligibility or DD214
(discharge papers).
|
|
FHA Only: Please bring a photo identification card.
|
|
Application Fees: Please be prepared to write a
check made payable to Pulaski Bank. Contact Charlie for the exact
amount.
